Instructions to Fill Form

• Use the form for request to get assistance in generation of FIDE ID, get AICF & MCA Annual Membership.

• Filling the form does not grantee your registration, membership or other request. A manual verification process is followed before we give any such assurance.

To ensure the digital security, and accepting documents, the E-Form is created using Google Forms and hence, the G-mail login is mandatory to fill the form. While, uploading/attaching the documents, ensure to submit valid (scan of original) documents either as image/pdf or photocopy while adhering to the size, quality, accessibility conditions.

• UID (Addhar Card) or equivalent alternative

• Birth Certificate or equivalent alternative

• ID Photo (600x480 pixel)

• Payment Proof
